What is the Lymphatic System

Understanding Your Lymphatic System and the Differences Between Lymphedema and Lipedema


When it comes to chronic swelling and fat distribution disorders, lymphedema and lipedema are often misunderstood

—and even misdiagnosed. Although they can appear similar at first glance,

these two conditions are fundamentally different in cause, presentation, and treatment.

Understanding their distinctions is essential for effective care, early intervention, and long-term management.

​What Is Lymphedema?

​

Lymphedema is a condition caused by damage or dysfunction in the lymphatic system, which is responsible for draining excess fluid (lymph) from tissues and returning it to the bloodstream (previously discussed). When this system becomes compromised—due to surgery, radiation, infection, trauma, or genetic factor-lymph can build up, causing swelling, typically in the arms or legs.

Key Signs of Lymphedema:

  • Swelling that often starts in the feet or hands and progresses upward

  • Skin that may feel tight, thick, or heavy

  • Risk of infection in the affected limb

  • Common after cancer treatment, especially breast or gynecological cancers

  • May affect only one limb

What Is Lipedema?

​

Lipedema, on the other hand, is a chronic fat disorder that affects almost exclusively women. It is believed to have a hormonal and genetic component, and it causes an abnormal accumulation of fat—most commonly in the hips, thighs, buttocks, and sometimes arms—but not the feet or hands.

Unlike ordinary weight gain, lipedema fat is painful and resistant to diet and exercise. Over time, it can become more severe, potentially leading to mobility issues and secondary lymphedema (known as lipo-lymphedema).
